TERMS OF SERVICE
AGREEMENT
BETWEEN USER AND PJR SALES AND SERVICE
The use of services from PJR Sales and Service.com, PJR Solutions.com,
Corner.net, Computerco-op.net , Cornernet.net (hereafter
referred to as "PJR") constitutes agreement to these terms.
PJR services is offered to you conditioned on your
acceptance without modification of the terms, conditions,
and notices contained herein. Your use of the PJR services constitutes your
agreement to all such terms, conditions, and notices.
MODIFICATION OF THESE TERMS OF SERVICE
PJR reserves the right to change the terms, conditions, and
notices under which the PJR service is offered, including
but not limited to the charges associated with the use of
the PJR service.
ACCOUNT SETUP/ EMAIL ON FILE
We will setup your account after we have received payment
and we and/or our payment partner(s) have screened the
order(s) in case of fraud. It is your responsibility to
provide us with an email address which is not @ the
domain(s) you are signing up under. If there is ever an
abuse issue or we need to contact you, the primary email
address on file will be used for this purpose. It is your
responsibility to ensure the email address on file is
current or up to date at all times. If you have a domain
name registered with PJR, it is your responsibility to
ensure that the contact information for your domain account
and your actual domain name(s) is correct and up-to-date.
PJR is not responsible for a lapsed registration due
to outdated contact information being associated with the
domain. If you need to verify or change this information,
you should contact our sales team via email. Providing false
contact information of any kind may result in the
termination of your account. In dedicated server purchases
or high risk transactions, it will be necessary to provide
government issued identification and possibly a scan of the
credit card used for the purchase. If you fail to meet these
requirements, the order may be considered fraudulent in
nature and be denied.
TRANSFERS
Our transfers team will make every effort to help you move
your site to us. However, transfers are provided as a
courtesy service, and we can not make guarantees regarding
the availability, possibility, or time required to complete
an account transfer. Each hosting company is configured
differently, and some hosting platforms save data in an
incompatible or proprietary format, which may make it
extremely difficult if not impossible to migrate some or all
account data. We will try our best, but in some cases we may
be unable to assist you in a transfer of data from an old
host.
The free transfer services are available for 30 days from
your sign up date. Transfers outside of the 30 day free
period will incur a charge; please contact a member of the
transfers department with specific details to receive a
price quote.
LINKS TO THIRD PARTY SITES
The PJR Web Site may contain links to other Web Sites
("Linked Sites"). The Linked Sites are not under the control
of PJR and PJR is not responsible for the contents of any
Linked Site, including without limitation any link contained
in a Linked Site, or any changes or updates to a Linked
Site. PJR is not responsible for web casting or any other
form of transmission received from any Linked Site. PJR is
providing these links to you only as a convenience, and the
inclusion of any link does not imply endorsement by PJR
of the site or any association with its operators.
NO UNLAWFUL OR PROHIBITED USE
As a condition of your use of the PJR Web Site, you warrant
to PJR that you will not use the PJR service for any purpose
that is unlawful or prohibited by these terms, conditions,
and notices. You may not use the PJR Web Site in any manner
which could damage, disable, overburden, or impair the PJR
Web Site or interfere with any other party's use and
enjoyment of the PJR Web Site. You may not obtain or attempt
to obtain any materials or information through any means not
intentionally made available or provided for through the PJR
service.
USE OF COMMUNICATION SERVICES
The PJR Web Site may contain bulletin
board services, chat areas, news groups, forums,
communities, personal web pages, calendars, and/or other
message or communication facilities designed to enable you
to communicate with the public at large or with a group
(collectively, "Communication Services"), you agree to use
the Communication Services only to post, send and receive
messages and material that are proper and related to the
particular Communication Service. By way of example, and not
as a limitation, you agree that when using a Communication
Service, you will not:
Defame, abuse, harass, stalk, threaten or otherwise violate
the legal rights (such as rights of privacy and publicity)
of others.
Publish, post, upload, distribute or disseminate any
inappropriate, profane, defamatory, infringing, obscene,
indecent or unlawful topic, name, material or information.
Upload files that contain software or other material
protected by intellectual property laws (or by rights of
privacy of publicity) unless you own or control the rights
thereto or have received all necessary consents.
Upload files that contain viruses, corrupted files, or any
other similar software or programs that may damage the
operation of another's computer.
Advertise or offer to sell or buy any goods or services for
any business purpose, unless such Communication Service
specifically allows such messages.
Conduct or forward surveys, contests, pyramid schemes or
chain letters.
Download any file posted by another user of a Communication
Service that you know, or reasonably should know, cannot be
legally distributed in such manner.
Falsify or delete any author attributions, legal or other
proper notices or proprietary designations or labels of the
origin or source of software or other material contained in
a file that is uploaded.
Restrict or inhibit any other user from using and enjoying
the Communication Services.
Violate any code of conduct or other guidelines which may be
applicable for any particular Communication Service.
Harvest or otherwise collect information about others,
including e-mail addresses, without their consent.
Violate any applicable laws or regulations.
Using a shared account as a backup/storage device is not
permitted, with the exception of one cPanel backup of the
same account. Please do not take backups of your backups.
Examples of unacceptable material on all Shared and
Dedicated servers include:
• Topsites
• IRC Scripts/Bots
• Proxy Scripts/Anonymizers
• Pirated Software/Warez
• Image Hosting Scripts (similar to Photobucket or Tinypic)
• AutoSurf/PTC/PTS/PPC sites
• IP Scanners
• Bruteforce Programs/Scripts/Applications
• Mail Bombers/Spam Scripts
• Banner-Ad services (commercial banner ad rotation)
• File Dump/Mirror Scripts (similar to rapidshare)
• Commercial Audio Streaming (more than one or two streams)
• Escrow/Bank Debentures
• High-Yield Interest Programs (HYIP) or Related Sites
• Investment Sites (FOREX, E-Gold Exchange, Second
Life/Linden Exchange, Ponzi, MLM/Pyramid Scheme)
• Sale of any controlled substance without prior proof of
appropriate permit(s)
• Prime Banks Programs
• Lottery/Gambling Sites
• MUDs/RPGs/PBBGs
• Hateful/Racist/Harassment oriented sites
• Hacker focused sites/archives/programs
• Sites promoting illegal activities
• Forums and/or websites that distribute or link to warez/pirated/illegal
content
• Bank Debentures/Bank Debenture Trading Programs
• Fraudulent Sites (Including, but not limited to sites
listed at aa419.org & escrow-fraud.com)
• Mailer Pro
• Broadcast or Streaming of Live Sporting Events (UFC,
NASCAR, FIFA, NFL, MLB, NBA, WWE, WWF, etc)
PAYMENT INFORMATION
You agree to supply appropriate payment for the services
received from PJR, in advance of the time period during
which such services are provided. You agree that until and
unless you notify PJR of your desire to cancel any or all
services received, those services will be billed on a
recurring basis. This includes when accounts are suspended.
When an account's invoices are paid outside of the 10 day
grace period, invoices will not be automatically generated.
However, after the customer notifies PJR of his or intent to
resume service, invoices will be manually generated and
payment expected for the time the account was suspended.
Cancellations must be done in writing via the cancellation
form provided. Once we receive your cancellation and have
confirmed all necessary information with you via e-mail, we
will inform you in writing (typically email) that your
account has been canceled. Your cancellation confirmation
will contain a ticket/tracking number in the subject for
your reference, and for verification purposes. You should
immediately receive an automatic "Your request has been
received..." email with a tracking number. An employee will
confirm your request (and process your cancellation) shortly
thereafter. If you do not hear back from us, or do not
receive the automatic confirmation email within a few
minutes after submitting your cancellation, please contact
us immediately via phone. We require that cancellations of
service are done through the online form to (a) confirm your
identity, (b) confirm in writing you are prepared for all
files/emails to be removed, and (c) document the request.
This process reduces the likelihood of mistakes,
fraudulent/malicious requests, and ensures you are aware
that the files, emails, and account may be removed
immediately after a cancellation request is processed.
As a client of PJR, it is your responsibility to ensure that
your payment information is up to date, and that all
invoices are paid on time. You agree that until and unless
you notify PJR of your desire to cancel any or all services
received (by the proper means listed in the appropriate
section of the Terms of Service), those services will be
billed on a recurring basis, unless otherwise stated in
writing by PJR. PJR reserves the right to bill your credit
card or billing information on file with us. PJR provides a
10 day grace period from the time the invoice is generated
and when it must be paid. Any invoice that is outstanding
for 10 days and not paid will result in a $10 late fee
and/or an account suspension until account balance has been
paid in full. The $10 late fee is applied in addition to
whatever else is owed to PJR for services rendered. Access
to the account will not be restored until payment has been
received.
It is the customer's responsibility to notify our sales team
via email at
sales@corner.net after paying for a domain renewal and
to ensure the email is received and acted upon. Domain
renewal notices and invoices are provided as a courtesy
reminder and PJR cannot be held responsible for failure to
renew a domain or failure to notify a customer about a
domain's renewal.
Invoices that have been paid more than once with multiple
Paypal Subscriptions can only be added as credit towards the
account and cannot be refunded via Paypal. If you require

PJR reserves the right to change the monthly payment amount
and any other charges at anytime.
CANCELLATIONS AND REFUNDS
PJR reserves the right to cancel, suspend, or otherwise
restrict access to the account at any time with or without
notice.
Any abuse of our staff in any medium or format will result
in the suspension or termination of your services.
Customers may cancel at any time via a cancellation form.
PJR gives you an unconditional 30 day money back guarantee
on managed shared hosting for any customer who paid the
first invoice with a credit card or with Paypal.
The following methods of payments are non-refundable, and
refunds will be posted as credit to the hosting account:
• Checks
• Money orders
There are no refunds on dedicated servers, administrative
fees, install fees for custom software, or domain name
purchases. Refund requests for shared and reseller accounts
after the initial 30 days will be refunded on a prorated
basis of any unused time.
Only first-time accounts are eligible for a refund. For
example, if you've had an account with us before, canceled
and signed up again, you will not be eligible for a refund
or if you have opened a second account with us.
Violations of the Terms of Service will waive the refund
policy.
RESOURCE USAGE
User may not:
• Use 25% or more of system resources for longer then 90
seconds. There are numerous activities that could cause such
problems; these include: CGI scripts, FTP, PHP, HTTP, etc.
• Run stand-alone, unattended server-side processes at any
point in time on the server. This includes any and all
daemons, such as IRCD.
• Run any type of web spider or indexer (including Google
Cash / AdSpy) on shared servers.
• Run any software that interfaces with an IRC (Internet
Relay Chat) network.
• Run any bit torrent application, tracker, or client. You
may link to legal torrents off-site, but may not host or
store them on our shared servers.
• Participate in any file-sharing/peer-to-peer activities
• Run any gaming servers such as counter-strike, half-life,
battlefield1942, etc
• Run cron entries with intervals of less than 15 minutes.
Users are strongly recommended to:
When using PHP include functions for including a local file,
include the local file rather than the URL. Instead of
include("http://yourdomain.com/include.php") use
include("include.php")
To help reduce usage, do not force html to handle
server-side code (like php and shtml).
Only use https protocol when necessary; encrypting and
decrypting communications is noticeably more CPU-intensive
than unencrypted communications.
BACKUP LIMIT
Any shared account using more than 20 gigs of disk space
will be removed from our off site weekly backup with the
exception of Databases continuing to be backed up. All data
will continue to be mirrored to a secondary drive which
helps protect against data loss in the event of a drive
failure.
BANDWIDTH USAGE
You are allocated a monthly bandwidth allowance. This
allowance varies depending on the hosting package you
purchase. Should your account pass the allocated amount we
reserve the right to suspend the account until the start of
the next allocation, suspend the account until more
bandwidth is purchased at an additional fee, suspend the
account until you upgrade to a higher level of package,
terminate the account and/or charge you an additional fee
for the overages. Unused transfer in one month cannot be
carried over to the next month.
MONEY BACK GUARANTEE
On dedicated servers and collocation no full refund will be
honored -- the 30 day money back guarantee does not apply.
We reserve the right to refund a prorated amount or no
refund at all.
Each of PJR's managed shared servers carry a 30 day
unconditional money back guarantee on them. If you are not
completely satisfied with our services within the first 30
days of your service, you will be given a full refund of the
contract amount. Remember, this is only for monthly shared
packages and does not apply to dedicated servers,
administrative fees, install fees for custom software, or
domain name purchases.
PJR will not activate new orders or activate new packages
for customers who have an outstanding balance with PJR. For
a new order to be setup or a new package to be activated,
you must have a balance of $0.00, unless otherwise stated by
PJR in writing.
UPTIME GUARANTEE
If your shared server has a physical downtime that is
not within the 99.9% uptime you may receive one month of
credit on your account. Approval of the credit is at the
discretion of PJR dependent upon justification provided.
Third party monitoring service reports may not be used for
justification due to a variety of factors including the
monitor's network capacity/transit availability. The uptime
of the server is defined as the reported uptime from the
operating system and the Apache Web Server which may differ
from the uptime reported by other individual services. To

with justification. All requests must be made in writing via
email. Uptime guarantees only apply to shared solutions.
Dedicated servers are covered by a network guarantee in
which the credit is prorated for the amount of time the
server is down which is not related to our uptime guarantee.
SPAM POLICY:
PJR does not authorize any of its clients to distribute
unsolicited bulk emails. In addition, Internet email sent,
or caused to be sent, to or through the PJR servers that
makes use of or contains invalid or forged headers, invalid
or non-existent domain names or other means of deceptive
addressing will be deemed to be counterfeit. Any attempt to
send or cause such counterfeit email to be sent to or
through the PJR servers is unauthorized. PJR reserves the
right to take all legal and technical steps available to
prevent unsolicited bulk email or other unauthorized email
from entering or leaving PJR's servers.
PJR has no obligation to monitor the Communication Services.
However, PJR reserves the right to review materials posted
to a Communication Service and to remove any materials in
its sole discretion. PJR reserves the right
to terminate your access to any or all of the Communication
Services at any time without notice for any reason
whatsoever.
ADULT and ILLEGAL MATERIAL:
Pornographic, sex-oriented, threatening or illegal material
is prohibited on any PJR account. This includes any content
that promotes any sexual or illegal content; such as via
external links. Any use of the PJR servers to display, store or distribute such
material will result in account termination.
PJR reserves the right at all times to disclose any
information as necessary to satisfy any applicable law,
regulation, legal process or governmental request, or to
edit, refuse to post or to remove any information or
materials, in whole or in part, in PJR's sole discretion.
Always use caution when giving out any personally
identifying information about yourself or your children in
any Communication Service. PJR does not control or endorse
the content, messages or information found in any
Communication Service and, therefore, PJR specifically
disclaims any liability with regard to the Communication
Services and any actions resulting from your participation
in any Communication Service. Managers and hosts are not
authorized PJR spokespersons, and their views do not
necessarily reflect those of PJR.
Materials uploaded to a Communication Service may be subject
to posted limitations on usage, reproduction and/or
dissemination. You are responsible for adhering to such
limitations if you download the materials.
MATERIALS PROVIDED TO PJR OR POSTED AT
ANY PJR WEB SITE
PJR does not claim ownership of the materials you provide to
PJR (including feedback and suggestions) or post, upload,
input or submit to any PJR Web Site or its associated
services (collectively "Submissions"). However, by posting,
uploading, inputting, providing or submitting your
Submission you are granting PJR, its
affiliated companies and necessary sublicense's permission
to use your Submission in connection with the operation of
their Internet businesses including, without limitation, the
rights to: copy, distribute, transmit, publicly display,
publicly perform, reproduce, edit, translate and reformat
your Submission; and to publish your name in connection with
your Submission.
No compensation will be paid with respect to the use of your
Submission, as provided herein. PJR is under no obligation
to post or use any Submission you may provide and may remove
any Submission at any time in PJR's sole discretion.
By posting, uploading, inputting, providing or submitting
your Submission you warrant and represent that you own or
otherwise control all of the rights to your Submission as
described in this section including, without limitation, all
the rights necessary for you to provide, post, upload, input
or submit the Submissions.
DATA STORAGE:
Your use of this service is at your sole risk. Our backup
service is ran on Sunday of each week, overwrites any of our
previous backups made, and only one week of backups are
kept. This service is provided to you as a courtesy. PJR is not
responsible for any data stored on our servers which becomes
corrupted or lost. It is always the client's responsibility
to maintain current backups of their own data in a location
that is separate and secure from the PJR network.
COUPONS
Discounts and coupon codes are reserved for first-time
accounts *or first-time customers* only and may not be used
towards the purchase of a domain registration unless
otherwise specified. If you have signed up using a
particular domain, you may not resign up for that domain
using another coupon at a later date. Any account found in
violation of these policies will be reviewed by our sales
department and the appropriate invoices will be added to the
account. Coupon abuse will not be tolerated and may result
in the suspension or termination of the account. Coupons or
discounts are only valid towards the initial purchase, and
do not affect the renewal or recurring price.
LIABILITY DISCLAIMER
THE INFORMATION, SOFTWARE, PRODUCTS, AND SERVICES INCLUDED
IN OR AVAILABLE THROUGH THE PJR WEB SITE MAY INCLUDE
INACCURACIES OR TYPOGRAPHICAL ERRORS. CHANGES ARE
PERIODICALLY ADDED TO THE INFORMATION HEREIN. PJR AND/OR ITS
SUPPLIERS MAY MAKE IMPROVEMENTS AND/OR CHANGES IN THE PJR
SERVICE AT ANY TIME. ADVICE RECEIVED VIA THE PJR SERVICE SHOULD NOT BE RELIED UPON FOR
PERSONAL, MEDICAL, LEGAL OR FINANCIAL DECISIONS AND YOU
SHOULD CONSULT AN APPROPRIATE PROFESSIONAL FOR SPECIFIC
ADVICE TAILORED TO YOUR SITUATION.
PJR AND/OR ITS SUPPLIERS MAKE NO
REPRESENTATIONS ABOUT THE SUITABILITY, RELIABILITY,
AVAILABILITY, TIMELINESS, AND ACCURACY OF THE INFORMATION,
SOFTWARE, PRODUCTS, SERVICES AND RELATED GRAPHICS CONTAINED
ON THE PJR WEB SITE FOR ANY PURPOSE. TO
THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, ALL SUCH
INFORMATION, SOFTWARE, PRODUCTS, SERVICES AND RELATED
GRAPHICS ARE PROVIDED "AS IS" WITHOUT WARRANTY OR CONDITION
OF ANY KIND. PJR AND/OR ITS SUPPLIERS
HEREBY DISCLAIM ALL WARRANTIES AND CONDITIONS WITH REGARD TO
THIS INFORMATION, SOFTWARE, PRODUCTS, SERVICES AND RELATED
GRAPHICS, INCLUDING ALL IMPLIED WARRANTIES OR CONDITIONS OF
MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE AND
NON-INFRINGEMENT.
PJR WILL NOT BE RESPONSIBLE FOR A
FAILURE TO RECOGNIZE OR REMOVE ANY SITES THAT MAY BE
ILLEGAL, HARMFUL OR OTHERWISE OBJECTIONABLE. PJR MAKES
NO REPRESENTATIONS AS TO THE ACCURACY OR ANY OTHER ASPECT OF
INFORMATION CONTAINED IN OUR, OUR CLIENTS', OR ANY LINKED
WEBSITES. IN NO EVENT WILL PJR, ITS RELATED
PARTNERSHIPS, OFFICERS, OWNERS OR EMPLOYEES THEREOF BE
LIABLE TO YOU OR ANYONE ELSE FOR ANY DECISION MADE OR ACTION

INCLUDING, WITHOUT LIMITATION, DAMAGES FOR LOSS OF USE, DATA
OR PROFITS, ARISING OUT OF OR IN ANY WAY CONNECTED WITH THE
USE OR PERFORMANCE OF THE PJR WEB SITE,
WITH THE DELAY OR INABILITY TO USE THE PJR WEB SITE OR RELATED SERVICES, THE PROVISION
OF OR FAILURE TO PROVIDE SERVICES, OR FOR ANY INFORMATION,
SOFTWARE, PRODUCTS, SERVICES AND RELATED GRAPHICS OBTAINED
THROUGH THE PJR WEB SITE, OR OTHERWISE
ARISING OUT OF THE USE OF THE PJR WEB
SITE, WHETHER BASED ON CONTRACT, TORT, NEGLIGENCE, STRICT
LIABILITY OR OTHERWISE, EVEN IF PJR OR
ANY OF ITS SUPPLIERS HAS BEEN ADVISED OF THE POSSIBILITY OF
DAMAGES. BECAUSE SOME STATES/JURISDICTIONS DO NOT ALLOW THE
EXCLUSION OR LIMITATION OF LIABILITY FOR CONSEQUENTIAL OR
INCIDENTAL DAMAGES, THE ABOVE LIMITATION MAY NOT APPLY TO
YOU. IF YOU ARE DISSATISFIED WITH ANY PORTION OF THE
PJR WEB SITE, OR WITH ANY OF THESE
TERMS OF SERVICE, YOUR SOLE AND EXCLUSIVE REMEDY IS TO
DISCONTINUE USING THE PJR WEB SITE.
INDEMNIFICATION
PJR'S CLIENTS AGREE TO DEFEND,
INDEMNIFY, SAVE AND HOLD PJR HARMLESS
FROM ANY AND ALL DEMANDS, LIABILITIES, LOSSES, COSTS AND
CLAIMS, INCLUDING REASONABLE ATTORNEY'S FEES ASSOCIATED WITH
PJR'S DEVELOPMENT AND HOSTING OF THEIR
WEBSITE(S). THIS INCLUDES LIABILITIES ASSERTED AGAINST
PJR, ITS SUBCONTRACTORS, ITS AGENTS,
ITS CLIENTS, OFFICERS, AND EMPLOYEES, THAT MAY ARISE OR
RESULT FROM ANY SERVICE PROVIDED OR PERFORMED OR AGREED TO
BE PERFORMED OR ANY PRODUCT SOLD BY THE CLIENT, ITS AGENTS,
EMPLOYEES OR ASSIGNS. CLIENTS ALSO AGREE TO DEFEND,
INDEMNIFY AND HOLD HARMLESS PJR AGAINST
LIABILITIES ARISING OUT OF ANY INJURY TO PERSON OR PROPERTY
CAUSED BY ANY PRODUCTS OR SERVICES SOLD OR OTHERWISE
DISTRIBUTED OVER THE CLIENT'S WEBSITE. THIS INCLUDES BUT IS
NOT LIMITED TO INFRINGING ON THE PROPRIETARY RIGHTS OF A
THIRD PARTY, COPYRIGHT INFRINGEMENT, AND DELIVERING ANY
DEFECTIVE PRODUCT OR MISINFORMATION WHICH IS DETRIMENTAL TO
ANOTHER PERSON, ORGANIZATION, OR BUSINESS.

TERMINATION/ACCESS RESTRICTION
In addition to CANCELLATION AND REFUNDS,
PJR reserves the right, in its sole discretion, to terminate
your access to the PJR Web Site and the related services or
any portion thereof at any time, without notice. GENERAL To
the maximum extent permitted by law, this agreement is
governed by the laws of the State of Maryland, the State of
Texas, and the United States of America and
you hereby consent to the exclusive jurisdiction and venue
of courts in Baltimore County, Maryland, U.S.A. in all
disputes arising out of or relating to the use of the PJR
service. Use of the PJR service is unauthorized in any
jurisdiction that does not give effect to all provisions of
these terms and conditions, including without limitation
this paragraph. You agree that no joint venture,
partnership, employment, or agency relationship exists
between you and PJR as a result of this agreement or use of
the PJR service. PJR's performance of this agreement is
subject to existing laws and legal process, and nothing
contained in this agreement is in derogation of PJR's right
to comply with governmental, court and law enforcement
requests or requirements relating to your use of the PJR
service or information provided to or gathered by PJR with
respect to such use. If any part of this agreement is
determined to be invalid or unenforceable pursuant to
applicable law including, but not limited to, the warranty
disclaimers and liability limitations set forth above, then
the invalid or unenforceable provision will be deemed
superseded by a valid, enforceable provision that most
closely matches the intent of the original provision and the
remainder of the agreement shall continue in effect. Unless
otherwise specified herein, this agreement constitutes the
entire agreement between the user and PJR with respect to
the PJR service and it supersedes all prior or
contemporaneous communications and proposals, whether
electronic, oral or written, between the user and PJR with
respect to the PJR service. A printed version of this
agreement and of any notice given in electronic form shall
be admissible in judicial or administrative proceedings
based upon or relating to this agreement to the same extent
and subject to the same conditions as other business
documents and records originally generated and maintained in
printed form. It is the express wish to the parties that
this agreement and all related documents be drawn up in
English.

notifications of claimed copyright infringement under United
States copyright law should be sent to Service Provider's
Designated Agent. ALL INQUIRIES NOT RELEVANT TO THE
FOLLOWING PROCEDURE WILL RECEIVE NO RESPONSE. See Notice and
